New Student Enrollment Guidelines

Documents needed at the time of registration:

1. Certified copy of the child's birth certificate.
2. Complete immunization records (REQUIRED Shots) - Click here to view Immunization Requirements for Preschool – Gr 12.
3. Name and address of previous school.
4. Proof of current residency - Utility bill or similar document which shows the current address of service.

• You will also be asked to fill out a Student Information Form.

• If you do not already have a certified copy of the birth certificate, the address to send for one in the state of Idaho is Vital Statistics, P.O. Box 83720, Boise, ID  83720-0036.  For other states, contact the state capital, your school office, or the LakePend OreilleSchool District office for addresses.

• Idaho law requires that students provide complete immunization documentation (or an Idaho Conditional Admission to School form has been submitted and accepted) in order to attend a public school in the state of Idaho.  Until all paperwork is complete and all required documents are provided, we will not be able to place students on a class list.

• Children entering kindergarten must be 5 years of age on or before September 1st.  For enrollment into first grade, a child must be 6 years of age on or before the first day of September.  There are no exceptions under Idaho law for exceptionally mature students to enter school early.
